Medicines in School
If your child has been prescribed medication that must be taken during school hours, please complete the form below and hand it in to the school office, along with the medication.
If your child has a condition that requires ongoing medication, such as asthma, eczema or allergies, we must keep a unit of the medicine in school for use. Two units of Epipens are required, one for the classroom and one for the office.
Please send these medicines into school with a copy of the 'Request for school to give Prescribed Medication' form and an Individual Healthcare Plan (IHP) completed for your child. If your child has asthma, you should also complete the related forms below.
All medicines should be sent in through the school office so that we have a record of the type of medicine, its dosage and use and its expiry date.
